Instructions
- This demo contains a subset of items from the complete Cultural Assessment Tool. The instructions listed here apply to the complete survey.
- Try to be honest and candid in expressing your views. Your individual responses to the survey will be completely anonymous.
- The survey contains 3 sections and should take approximately 30 to 40 minutes to complete. At any time you have the option to save your responses and continue later.
- Section 1 consists of 232 items. Read each item carefully and select the response that best reflects your opinion. The possible responses range from "Strongly Disagree" to "Strongly Agree." Please try to answer all of the items. If you find that you are unable to rate an item, you should mark the "Unable to Rate" box.
- Section 2 is a checklist of characteristics and section 3 includes several short answer questions.
Ergonomic Relief:
If you prefer, you can use the keyboard instead of the mouse to complete the survey, but only if you are using Internet Explorer.
Keyboard Controls:
- Tab - Advance to the next question
- Left and Right Arrows - Select an answer in the current question
- Return - Advance to the next page of questions
Note: You may need to press Tab several times before it is advanced to the first question on a page. It may be easier to use the mouse to answer the first question on each page, and use the keyboard for the rest of the page.
